The College of Physicians & Surgeons of Ontario requires physicians to complete and submit third-party reports within sixty (60) days. Ensure that you complete your portion of the form and sign the release beforehand; otherwise, your physician will not be able to complete it. Once the form is completed, we will contact you. If you have not heard from us after thirty (30) days, please call to check the status of your form.
